Connected Communities Implementation Grantees and Collaborators

The Connected Communities Implementation Grant program is designed to support organizations in demonstrating community collaboration using health IT to address real-world, practical and measurable healthcare needs among the patients they serve and to improve care coordination.

On November 1st, 2016, MeHI hosted the second annual Grantee Forum, which included educational presentations, networking, and showcased Connected Communities' grantees' projects.
