Implementation Grant Solicitation

MassTech, on behalf of MeHI, has issued this Request for Proposals for the Connected Communities Implementation Grant Solicitation (RFP No. 2015-MEHI-05). The RFP solicits responses from eligible Treatment, Payment, and healthcare Operation (TPO) organizations in Massachusetts. Applicants will be required to meet program milestones that demonstrate community collaboration around a real-world, practical, measurable care coordination need. This program builds upon previous investments in Health IT and is intended to incentivize inter-organizational use of Health IT to strengthen care coordinating workflows and information pathways through a set of progressive and tiered milestones.

Application Process

The Grant Program will use a two-tier review process: 1) pre-application tier and 2) application tier. This approach allows MeHI to invite broad participation with minimal initial effort from prospective grantees. Selected pre-applications will be invited to submit full applications.

MeHI will award Implementation Grants to quality full applications. Grantees will need to complete a Transformation Plan and four milestones to earn all incentives (see “Grant Program Deliverables” table).

Full applications that show promise, but need modification to be of merit, may be awarded a Community Planning Award of $25,000 to complete only the Transformation Plan. If this results in a qualifying proposal, MeHI may also award the milestone portion of the Implementation Grant, contingent upon funds remaining.
